Planning Forms

There are various forms required when lodging an application. Some forms are available for download while others are available for completion online.

The following forms are available for download at the bottom of this page. These forms are in Acrobat format and must be printed out and completed by hand.

Development Application Form

Most types of building, subdivision and demolition work requires a Development Application (DA) to be submitted to Council for approval.

A Development Application form is required for the following types of consent, certificate or approval:

• Development Consent
• Combined DA/Construction Certificate
• Development Consent - Subdivision
• Subdivision Construction Certificate
• Complying Development
• Building Construction Certificate
• Development Application Modification
• Integrated Development Approval
• Other Approval under s68 of the Local Government Act 1993

Applicants Checklist

This checklist will assist you with submitting a Development Application form with the required information.

Subdivision Certificate Application Form

You need a subdivision certificate to have your plan of subdivision registered with Land and Property Information NSW.

Sewage Management System Application Form

This application form is required for approval to install/operate/alter a Sewage Management Facility.

Appointment of Parkes Shire Council as the Principal Certifying Authority

This form is required to be submitted to Parkes Shire Council if you wish to engage Parkes Shire Council as the Principal Certifying Authority.

Online Forms

The following forms are available for completion online.

Notice of Commencement of Building or Subdivision Work

This form is required to be submitted to Parkes Shire Council at least two (2) days prior to the commencement of any works being carried out.
