What You Need to Include

To help us assess your claim, please attach:

• Photos of the damage
• A copy of the quote or invoice for repairs or replacement
• Evidence that Council caused the alleged loss or damage through negligence
  Examples of evidence:
• • Reports from qualified service providers (e.g., engineer, plumber, pre-purchase inspection report)
  • Medical documentation (e.g., practitioner reports, hospital discharge summaries, treatment details)

• The burden of proving negligence rests with you as the person seeking compensation. Council cannot assist you in gathering evidence.
• Submitting this form does not guarantee compensation.
• All claims are assessed on their own merit through a formal review process, which may take approximately 6–8 weeks.

Here are some supporting documents that may be required when filling out an event application form:

Risk Assessment(PDF, 39KB)

Applicants may be required to complete a risk assessment prior to their event. This assessment identifies potential hazards and outlines the measures that will be implemented to ensure the event is delivered safely and responsibly.

Mayoral Request Form(PDF, 147KB)

If you would like the Mayor or Councillors to attend or participate in your event, a Mayoral Request Form must be completed and submitted for consideration.

Notice of Intention to Hold a Public Assembly(PDF, 68KB)

This form is required for any event that is open to the public. It ensures the event is formally registered and enables appropriate planning to support public safety and compliance.

Application for Use of Park(PDF, 395KB)

If your event is to be held in a public park, an Application for Use of Park must be completed and submitted with your event documentation. This enables Council to assess suitability and availability of the space,  as well as identify any additional requirements.

Section 138 - Application to Occupy Footpath & Public Street(PDF, 301KB)

This application must be submitted if your event requires the use, closure, or partial closure of a public road or footpath. It applies to activities such as parades, fun runs, street festivals, or  the installation of infrastructure such as stages and barriers that impact vehicle or pedestrian access.

Council approval must be granted before any works or road occupancy can commence.

This form is used to submit your event details for promotion on the Parkes Shire Council website. It should be completed once your event information is confirmed and you would like it advertised online.

You will need to provide details such as the event title, date, time, location, cost, contact information, ticket links, and an event image (if available).
