Create a Soundscape: Years 3-6

1. This event is exclusive for primary aged children in years K-2.
2. Attendees must be Library members. Membership to the Library is free.
3. All young people must be signed in and signed out by a responsible adult. The responsible adult must be available as an emergency contact throughout the event.
4. At check out, please disclose any allergies or conditions staff would need to be aware of in order to provide you with your best experience.
5. If you are unwell, please do not attend.
6. Please phone the Library and cancel your booking prior to the event.

7. By registering your child for the Parkes Shire Library School Holiday Activities, you acknowledge and agree that:

   1. You grant permission for your child to participate in the scheduled activities.
   2. You are the nominated primary adult responsible for your child and agree to remain contactable for the duration of the activity in case of emergency.

Photography and Media Consent

Please indicate whether you consent to your child being photographed and/or recorded during the activities. Images and recordings may be used by Parkes Shire Library for promotional purposes, including on digital platforms and social media.
